Start with the part of the burn that has nothing to do with temperature. She couldn't let go.
August 2023, a medical building in Phoenix, a meeting to get to. Jennifer Longdon put her hand on a metal railing that had been standing in full sun and it took the skin off her palm: second- and third-degree burns. She held on anyway. Letting go meant risking a fall out of her wheelchair onto pavement that had been cooking since morning, and Phoenix pavement in August does to a body exactly what the railing was already doing to her hand. So she stayed with the hand.
"It hurts like hell. This thing is on fire."
That's what she said afterward. Recovery, per the reporting, could take months.
She said it in present tense, describing the object rather than herself, while it was still burning her. She ran the arithmetic mid-injury, put a burned palm on one side and a fall on the other, and chose.
Longdon served in the Arizona legislature. She has a complete spinal cord injury at the fourth thoracic vertebra, which means, as a disability magazine puts it, no voluntary sensation or motor control from roughly mid-chest down. The burn landed on one of the few parts of her that can still report temperature. Six inches lower and that same steel would have registered as nothing whatsoever.
Whether a heat alert reached her that afternoon isn't in the record, and I'm not going to invent one. What the record does say is that she was headed into a medical building for a meeting, in a season when the National Weather Service was broadcasting daily and the city's cooled libraries were open. She was moving toward the indoors. She got hurt on the way.
Phoenix's heat response runs on a sequence four links long. A warning is issued. A person receives it. She recognizes it as being about her. She moves through the built environment to somewhere cooler. Longdon's afternoon snapped the fourth link, on a piece of hot steel that was simultaneously her route indoors and the thing that burned her.
Ten months later and about twenty miles northwest, the chain broke at the second link, and it looked nothing like a railing.
The air conditioner was running
Patricia Miletich was 70. She lived in a senior RV resort in El Mirage. She died in June 2024 with her power on and her air conditioner running, and investigators put a thermometer to the vent and measured air coming out of it between 109 and 117 degrees.
A neighbor told death investigators that Miletich had memory problems, that she had forgotten to pay bills before and lost service over it. Her brother Michael said afterward that she "wanted to be left alone and the family didn't know," according to reporting on her autopsy file.
"Memory problems" is a neighbor's phrase. No diagnosis appears anywhere in the published record. Miletich is not documented as having dementia, an intellectual disability, or a developmental disability, and nobody gets to promote a neighbor's observation into a clinical finding because it would make an argument tidier. What her file can carry is narrow and still useful: it is evidence about the comprehension link, the point in the sequence where a warning has to arrive and be understood as personally addressed before it becomes anything at all.
This publication has already pried open one gap in that file. "Has AC: Yes" sat in the space between a database field recording that a machine existed and a death certificate recording what the machine actually did. I want the link before the machine. Warnings went out across phones and radios and screens all over Maricopa County that June. Whether one of them got inside that RV, and if it did, whether it read as being about her, is not in the record and cannot be recovered now.
Which brings me to an absence, and the absence is the finding.
Across three of the hottest summers in the recorded history of an American city, I could not find one Phoenix-area resident with a documented intellectual or developmental disability describing their own experience of the heat in a public source. I went through local and national coverage of the 2022, 2023, and 2024 seasons, Maricopa County's annual heat mortality reports, Phoenix's heat response plan, the regional heat relief network's own site standards and the federal assessment of that network, and disability-related local reporting. The institutions positioned to hold such an account, meaning the city's disability advocacy organizations, the state's developmental disability services system, group home operators, people who testify before city and county bodies, have not put one into the published record.
On the cognitive side, what exists is fragments. Miletich entirely secondhand: a phrase from a neighbor, a regret from a brother, a thermometer at a vent. Concetta Davies, 79, bedbound and living with dementia, found by a caregiver in a home measuring 102 degrees during the 2023 heat wave; her account runs one sentence, and because she was bedbound her case welds cognitive and mobility dependence together instead of letting either be seen on its own. The closest thing to a scene-rich portrait of a person with a developmental disability I found was in a Phoenix New Times summer listicle: a young woman with Edwards syndrome, cerebral palsy, and epilepsy who could not walk or speak and, per her mother, did not sweat, her room and her equipment entirely dependent on air conditioning. Wrong county. Wrong year. That the best available account sits in a listicle outside the window is itself a measurement of something.
Disabled people enter the record as narrators when they can narrate. When the disability is precisely the capacity to interpret a warning and afterward explain what went wrong, the person enters as a case file instead: an age, an indoor temperature, a neighbor's recollection. The population most likely to break the chain at comprehension is the population least equipped to testify about the break.
What the injury does that the forecast can't see
Longdon can narrate, and what she describes runs well past railings.
A spinal cord injury cuts two systems at once. It interrupts the signals that carry skin temperature upward, and it interrupts the nerve traffic that tells sweat glands to open and blood vessels near the skin to widen. Below the injury, sweating can be reduced or absent, which takes away evaporative cooling. Vasodilation can be impaired, which takes away the body's ability to move heat from the inside out to the surface where it could be shed. International clinical standards describe the impairment as level-dependent: the higher the injury sits, the less functioning skin is left available for sweating, with dysfunction especially common at or above the sixth thoracic vertebra. Longdon's injury is at the fourth, and it is complete.
The railing was the visible half of that afternoon. The invisible half is a woman whose sweating surface has been drastically reduced standing outdoors in August in a city where the surfaces are themselves heat sources, holding on to one of them, with the parts of her that might have warned her she was overheating silenced by the same injury.
What the research will not hand anyone is a number.
In one controlled study, twenty men with high injuries went from a room at 27 degrees Celsius into a chamber at 35 for up to two hours, alongside nineteen uninjured controls. Thirteen of the twenty showed core temperature rises of at least half a degree. Seven didn't. Among the ones who responded, sweat production ran about a fifth of the controls'. A separate small experiment that ramped ambient temperature from 27.5 to 40 degrees across fifty minutes found no statistically significant core-temperature rise in either its spinal-injured participants or its controls. Different protocol, different duration, different mix of injuries. Not a refutation. A demonstration that no clean cutoff can be pulled out of what exists. A clinical review went hunting for heat-prevention guidance written specifically for spinal cord injury and found none anywhere, and noted that the widely repeated threshold around the eighth thoracic vertebra traces back to a handful of very small early studies rather than to anything replicated.
Which leaves something narrower than a threshold and harder to act on. Risk climbs as the injury level rises and the sweating surface shrinks, individual responses scatter, and no public warning product in this country is calibrated to any of it. On a 110-degree afternoon in Phoenix, Longdon's danger and her neighbor's danger are two different quantities, and every instrument either of them consults reports one number.
One forecast, two dangers
Put both people in front of the same afternoon, the same forecast and push notification, the same cooled library eight blocks away.
The National Weather Service office in Phoenix is not naive about this. Its heat guidance says outright that heat illness and death start in the mid-80s, that no single temperature defines danger in all circumstances, and that the lower tiers of its risk scale exist specifically to flag conditions that matter for unusually heat-sensitive people. Formal warnings, though, are held for the top of the scale, with high forecaster confidence behind them. Phoenix meteorologist Matt Salerno explained in July 2024 that because the city's baseline is already extreme, the temperature it takes to trigger a major-risk day moves around across the season, and that residents shouldn't be waiting for a warning at all, because ordinary summer days here carry real risk.
The city makes the same case in bodies. Phoenix's 2024 heat response plan counts 67 heat-associated deaths during the 98 days of the 2023 season when the daily high never even reached 110 degrees. That is why the response runs as tiers from May through September instead of as a switch wired to alerts.
I went looking for the argument this should have produced: a named clinician or disability advocate saying on the record that alert thresholds sit too high for people who cannot thermoregulate, and a named official defending where they sit. In the 2022 through 2024 record, it isn't there. What's there is two agencies working at different levels, with nobody contesting the gap on behalf of disabled residents. The closest analogue I found is five years older and about utility shutoffs rather than weather: in a 2019 proceeding before Arizona's utility regulators, consumer advocate John Coffman pushed for disconnection protection at 95 degrees while staff and commissioners weighed 105, and AARP Arizona's Steve Jennings argued the lower figure was the one that matched the public health evidence. Even that fight wasn't about disabled customers specifically.
Standing is what determines which arguments ever get started, and on this one nobody had it.
What the county writes down, and what it doesn't
Maricopa County counts its heat dead more rigorously than almost anywhere: 425 in 2022, 645 in 2023, 608 in 2024. Recent editions of the annual report add selected medical histories, dementia and schizophrenia among them. Here is what those reports tabulate, and what they don't:

The gap is not new and it is not closing. No disability field in 2022, none in the most recent report, which covers 2025. Worth reaching past the window to say that, because the field list didn't change.
The missing medication field builds its own particular blindness. CDC guidance for clinicians lists whole classes of drugs that reset a body's relationship to heat, and they don't all reset it the same direction. Antipsychotics can interfere with central temperature regulation and, through anticholinergic effects, shut down sweating. Topiramate, prescribed for seizures and migraine, can reduce sweating and cause dangerous overheating. Lithium levels can creep toward toxicity as a person dehydrates. Sedating drugs, benzodiazepines and opioids included, can blunt both thirst and the recognition that something has gone wrong. Then the guidance complicates itself: antidepressants in the SSRI and SNRI classes may increase sweating rather than suppress it, raising dehydration risk instead. CDC is explicit that a lot of this rests on expert opinion rather than trials. Psychiatric and antiseizure prescriptions are also common among adults with intellectual disability, not as treatment for the disability but because of coexisting epilepsy and psychiatric conditions, which means the medication blindness and the disability blindness settle over the same people.
Put that next to the toxicology tables and the shape of the county's knowledge comes clear. It can tell you with real precision what was in a body at the hour of death. It cannot tell you what had been setting that body's dangerous temperature all summer long.
The reports draw on death certificates and medical examiner files, so these facts may well be sitting in individual case narratives. They aren't tabulated. Untabulated means it can't be trended, mapped, or budgeted against. Longdon sat in the legislature. She has been in the rooms where budgets like this one get written and where somebody decides what the county will bother to watch. Her body is still not a field. Whatever the county learns about her physiology, or about Miletich's memory, it learns one file at a time and stores nowhere.
Side by side, the two summers describe a design specification. The intended user gets an alert on a device, reads it as addressed to her, and travels an accessible route to a cooled building. Longdon fell outside that spec at the route. Miletich, as far as anyone can reconstruct, fell outside it at the alert.
The two gaps have different owners, and the difference matters more than the symmetry does. Longdon's railing belongs to somebody. A property manager. A building code. A municipal heat office already thinking about shade structures and surface materials. There are people whose job description contains that rail.
Miletich's unreceived warning belongs to no one. No agency, no case manager, no line in any heat plan carries the task of noticing that an alert went out and did not land. Which is why the system's first notice of failure arrives afterward, the way it arrived for her and for Concetta Davies: from a neighbor, from a caregiver at the door, from a thermometer held up to an air conditioner that had been running the whole time.
