Seeger and Minderhoud (Nature, 2026) found that 90% of coastal hazard assessments peg sea level to geoid models rather than measured water height, understating exposure by 0.24 to 0.27 meters. Under one meter of rise, that discrepancy places tens of millions more people below projected flood lines than any current map shows. The Heat Disappears Into the Diagnosis Code

An overheated apartment sends a 74-year-old to the ER. The discharge record says fall, or renal injury, or cardiac event. The heat that caused it never reaches the billing code, which means it never reaches the dataset, which means it never reaches the public health budget. Smoke Reversed a Decade of Cleaner Air

Between 2000 and 2015, U.S. ozone levels dropped 0.65 ppb per year. Since 2015, wildfire smoke has flipped that trend to a rise of 0.13 ppb annually (Deng et al., Science, 2026). The regulatory data calls it an ozone exceedance. It doesn't say wildfire. Communities hundreds of miles from any fire perimeter absorb the lung damage without appearing in any wildfire impact assessment.

Flood Risk Maps Track Water, Not Poison

Cushing et al. (Nature Communications, 2025) identified 5,500 U.S. hazardous facilities at flood risk by 2100 under high emissions. Renters, households without vehicles, and linguistically isolated communities face disproportionately higher odds of living nearby. Standard flood maps measure water reaching a property line. They say nothing about what that water picks up on the way there, or who can leave when it arrives.

Groundwater Depletion

No Gauge on the Aquifer Until It's Gone

Two billion people live on ground that is actively sinking from aquifer depletion (UN University, 2026). Much of the resulting scarcity gets labeled drought, which makes it sound like weather. It often isn't. It's pumping schedules, water law, crop economics, and municipal growth draining the same finite supply. Without continuous monitoring, restrictions don't kick in until a well goes dry and replacement costs spike overnight.

Monitor Gaps

Where There's No Sensor, There's No Warning

Fifty-nine percent of U.S. counties have no air quality monitor at all (Roque et al., PNAS, 2025). Counties with higher poverty rates and larger Black and Hispanic populations are more likely to be unmonitored. When private sensors fill the void, they cluster in wealthier, whiter neighborhoods. The CDC's 2024 heat death count, 2,394, comes from certificates where someone wrote "heat." Excess-mortality research puts the real toll several times higher. Researchers estimated wildfire smoke drives roughly 17,000 strokes per year in the U.S. Each enters claims data as a stroke, nothing more.

What vanishes from the record vanishes from the budget. Cooling centers, filtration programs, outreach workers all compete for funding based on documented need. If the cause never made it onto paper, the need doesn't register.

The count gap: CDC's 2024 official heat deaths: 2,394. Chicago's 1995 heat wave alone: ~700 excess deaths, only 465 on certificates.

The smoke toll: An estimated 24,000 deaths/year and 17,000 strokes/year attributed to wildfire smoke PM2.5. No safe threshold found.

The coding gap: ICD-10 external-cause codes for heat exist but require active physician documentation. When a chart says "acute kidney injury" without noting temperature, coders can't attach the exposure.

Medication, untracked: LANTUS insulin degrades above 86°F and looks normal afterward. VENTOLIN HFA inhalers risk bursting above 120°F. No system tracks whether a July delivery, a power outage, or an evacuation breached those thresholds.
